Job Description:

Are you passionate about helping others achieve independence and stability? Join our dedicated team as a Case Manager for our Bridge to Home program, supporting individuals living in the community through a strength-based, housing-first approach.

What You’ll Do:

• Support clients with goal setting, budgeting, advocacy, and community integration.
• Maintain a manageable caseload (11:1 ratio) to ensure personalized attention.
• Collaborate with landlords, agencies, and other case managers to identify housing and support options.
• Assist with applications for housing, treatment, and income supports (AISH, Alberta Works).
• Maintain accurate documentation using the HMIS database and contribute to program evaluations.


Education Required:

What You Bring:

• A diploma/degree in Human Services (or related field).
• 2+ years’ experience in case management or supporting vulnerable populations.

  
Skills Requirements:

• Knowledge of the homeless-serving sector, mental health, and substance use.
• Strong interpersonal, communication, and problem-solving skills.
• A valid driver’s license, personal vehicle, and satisfactory criminal record check.
